Meet Dr. Fowler
Elena Fowler, MD, FAARM, ABAARM
Functional and Integrative MedicinE For Brain Health and Healthy Aging
I am a functional and integrative medicine physician practicing in Sonoma, California, specializing in brain health, memory concerns, cognitive performance, and healthy aging.
My story is unique because I combine Internal Medicine, Psychiatry, Functional Medicine, Brain Health, and Longevity Medicine. This is a rare combination.
I did not choose this work casually. I chose it because I have seen what happens when cognitive decline, metabolic dysfunction, and chronic inflammation are missed early, and I have also seen what is possible when we intervene with precision and intention.
As a board-certified physician trained in both Internal Medicine and Psychiatry, with advanced fellowship training in anti-aging and regenerative medicine, my clinical lens has always been both broad and deeply detailed.

I am trained to understand complex physiology, but I am equally committed to understanding the person sitting in front of me, their unique story, their stress load, their family history, their fears, and their goals.
I specialize in working with individuals who are noticing changes they cannot ignore: memory concerns, cognitive decline, reduced focus, persistent fatigue, mood shifts, hormonal imbalances, or metabolic dysfunction. Many are high-functioning professionals who refuse to accept “this is just aging.” Others have a strong family history of dementia and want to protect their future before symptoms ever appear. I take both groups seriously.
What makes my approach different is integration. I use the latest in advanced testing and personalized care plans to identify and address underlying contributors to cognitive and age-related decline. My background in psychiatry allows me to understand the emotional and cognitive dimensions of health. My internal medicine training grounds me in comprehensive medical evaluation and risk assessment. My advanced education in functional, metabolic, and longevity-focused medicine allows me to ask a deeper question: what is driving this pattern?
I do not separate the brain from the body. I do not separate mood from metabolism. I do not separate hormones from cognition.
When you work with me, we examine the full landscape — insulin resistance, inflammation, mitochondrial function, nutrient sufficiency, thyroid signaling, stress physiology, sleep architecture, cardiovascular risk markers, environmental exposures, and genetic predispositions. I use advanced laboratory testing not to overwhelm you with data, but to create clarity.
Many patients come to me after being told their labs are “normal,” yet they know something is not right. I take that seriously. I believe symptoms are signals, not inconveniences. My role is to decode them.
Care in my practice is not rushed. It is deliberate. I take the time to listen carefully, analyze thoughtfully, and explain clearly. Your treatment plan is never generic. It is built specifically for your physiology, your risk profile, and your long-term goals. That may include precision nutrition, metabolic optimization, hormone balancing, targeted supplementation, prescription management when appropriate, and structured lifestyle interventions that protect both cognitive and physical resilience.
For those with a family history of cognitive decline, prevention is not a passive strategy. It requires early detection, risk modification, and consistent recalibration. I help you understand where you stand and what you can do — practically and proactively — to shift your trajectory.
This work is deeply relational to me. I believe trust is foundational to healing. I do not see myself as someone who simply orders labs and writes prescriptions. I see myself as your partner in protecting what matters most: your clarity of mind, your strength of body, and your confidence in the years ahead.
My goal is not just to help you feel better temporarily. It is to help you remain mentally sharp, metabolically strong, and biologically resilient — building a sharper, stronger future with intention and evidence guiding every step.
